I consent to any emergency treatment necessary during the stay on the Adventure Playground. I therefore authorise the party leaders, to sign on my behalf, any form of written consent requested by a hospital authority or doctor, provided that the delay required to obtain my signature might be considered, in the opinion of the hospital authority or doctor concerned, likely to endanger my child’s health or safety.
Goldhill adventure playground is an “open access” centre. This means all children must be registered, there is no charge, and children are free to come and go as they please. While the children are on the playground our staff have a legal duty of care, which means they must act in the same way as a caring parent would. This does not extend to preventing children from leaving. As part of this duty of care we must ask you to make sure that any child under the age of 8 is accompanied to and from the playground by someone over the age of 16.
I understand that Goldhill Adventure Playground cannot take responsibility for any children under 5 years.
I agree to allow my child to have his/her photograph taken for inclusion in publicity for the Adventure Playground. I understand this would be at the play worker’s discretion.
